Health officials warn unvaccinated residents face growing risk as cases climb across the state.

Two residents of Lancaster County, Pennsylvania, have died after contracting measles, marking the first fatalities linked to the disease in the United States this year and the first recorded in the state in 35 years. Both victims were unvaccinated, according to the Pennsylvania Department of Health, which confirmed the deaths this week.

The losses underscore a troubling resurgence of a disease many Americans had come to view as a relic of the past. Measles was declared eliminated from the United States in 2000, but declining vaccination rates in recent years have opened the door for the virus to spread again through communities with pockets of low immunization coverage.

A Community Unprepared for Measles

Pennsylvania’s top health official acknowledged that decades without local cases left many residents unfamiliar with how severe the illness can become, and she urged the public to trust the safety and effectiveness of the combined measles, mumps and rubella vaccine as the strongest available defense.

Lancaster County has borne the brunt of the outbreak. State data show 393 confirmed cases as of Tuesday, with 86 diagnosed in just the past week alone. Seventy people across Pennsylvania have required hospitalization for complications tied to the virus, a toll that highlights how quickly the disease can escalate once it takes hold in an unvaccinated population.

Vaccine Hesitancy Fuels the Spread

The outbreak comes amid a broader national retreat from routine childhood immunizations. Federal data show that during the 2025-26 school year, roughly 4.2% of kindergartners nationwide, or about 155,000 children, had at least one vaccine exemption on file, up from 3.6% the year before.

The trend is visible locally as well. Lancaster County’s MMR vaccination rate among schoolchildren has slid from 95.5% in 2017 to 87.6% this year, a decline public health experts say has steadily chipped away at the community immunity needed to prevent outbreaks like the one now unfolding.

The timing has also drawn political scrutiny. Earlier this month, the White House issued an executive order aimed at reducing the number of vaccines recommended for children, a move that drew swift condemnation from pediatric and public health groups concerned it could accelerate the erosion of protection against preventable diseases.

National Numbers Tell a Bigger Story

Pennsylvania’s outbreak is part of a much larger pattern playing out across the country. The Centers for Disease Control and Prevention has tallied 2,777 measles cases nationwide so far this year, the highest annual total in 35 years. Roughly 7% of those infected, or 193 people, have needed hospital care since late August, and health officials note that the overwhelming majority of cases involve people who were not vaccinated.

Pennsylvania’s two deaths add to a small but sobering national count. Last year, measles claimed three lives in the U.S.: an adult in New Mexico and two young girls in Texas. Public health officials say those earlier deaths, like this week’s, share a common thread of forgone vaccination.

Why the MMR Vaccine Still Matters

Despite renewed anxiety around the virus, health experts emphasize that protection remains widely available and highly effective. The MMR vaccine, typically administered in two doses around ages 1 and 5, provides about 97% protection against measles, often for a person’s entire life.

As Pennsylvania’s outbreak continues to grow, state officials say the coming weeks will be critical in determining whether the virus can be contained or whether it will continue spreading through communities with lower immunization rates. For now, health authorities are leaning on a message they hope resonates beyond Lancaster County: vaccination remains the most reliable shield against a disease that, until recently, most Americans never expected to see again.
