A common sign of gum problems may be connected to long term brain health, according to new research from Japan.
A decade long study involving nearly 1,400 adults found that people with more extensive gum bleeding were more likely to develop dementia during the study period than those with fewer signs of bleeding. The association was also observed specifically with Alzheimer’s disease, adding to a growing body of research examining the relationship between oral health and cognitive health.
The findings, published in The Journal of Clinical Periodontology, do not establish that gum problems cause dementia. Instead, they identify an association that researchers say warrants additional investigation.
Researchers followed participants for 10 years
The study followed adults in Japan for approximately a decade, examining several measures of periodontal health alongside participants’ neurological outcomes.
Among the participants, those with the greatest amount of gum bleeding had a 56% higher likelihood of developing dementia from any cause compared with people who had the least bleeding. Their likelihood of developing Alzheimer’s disease was also 61% higher.
The researchers identified additional associations involving the condition of the gums.
Participants with more pronounced gum recession had a 59% higher risk of developing dementia from any cause. Those with deeper spaces between the gums and teeth had a 72% higher risk.
These measurements can provide information about the condition of gum tissue and may indicate ongoing periodontal problems.
The study recorded 267 cases of dementia from all causes during the follow up period, including 51 cases of vascular dementia. The relatively smaller number of some dementia types makes it difficult to determine whether particular oral health measures are associated with specific forms of the condition.
Inflammation may help explain the connection
One possible explanation involves inflammation.
Bleeding and receding gums can occur alongside periodontal inflammation, which may affect more than the tissues surrounding the teeth. Researchers are examining whether inflammatory activity elsewhere in the body could influence processes involved in brain health.
One theory is that persistent inflammation may affect the blood brain barrier, a protective system that helps regulate what enters brain tissue from the bloodstream. Changes to that barrier could potentially allow inflammatory substances to reach the brain and contribute to processes associated with neurological damage.
That possibility remains under investigation, however. The current research cannot establish a direct biological pathway between gum disease and dementia.
Another possibility involves microorganisms found in the mouth. Previous experimental research has examined whether certain oral bacteria can travel beyond the mouth and affect other parts of the body, including the brain.
Animal research has provided some evidence that oral infection can be followed by changes in brain tissue and increased production of proteins associated with Alzheimer’s disease. Those findings are useful for understanding possible mechanisms but cannot demonstrate that the same process occurs in humans.
More research is needed
The latest study adds to previous research examining connections between dental health, inflammation and cognitive decline. However, the researchers emphasize that the findings need to be tested in larger and more diverse groups of people.
Because the study was observational, researchers could identify patterns but could not determine whether gum problems directly contributed to the development of dementia. Other factors may also influence both periodontal health and cognitive outcomes.
Still, the findings highlight why maintaining good oral health can be an important part of overall wellness.
Regular dental visits can help identify changes in the gums and teeth, while routine brushing and cleaning between the teeth can help maintain oral health. Persistent bleeding, gum recession or other changes should be discussed with a dental professional.
For researchers, the bigger question is whether improving periodontal health could eventually play a role in reducing the risk of cognitive decline. More long term research will be needed to determine whether better gum health changes dementia risk or whether the two conditions are connected through other underlying factors.
